Help me with my Kidney Transplant

I'm a person with a lot of illnesses but still fighting to survive. I'm type 1 diabetic with chronic asthma, hypertension, vitreo retinopathy (both eyes), had cerebellar stroke twice, and the most recent is kidney failure, which confines me to a dialysis treatment couple of days a week. Having a kidney transplant will better improve my health and qay of life. Please help me make it a reality
